Almost nobody in care work has been taught to write a note. You were shown the system, told to document everything, and left to copy whatever the last person wrote. Which is how a whole sector ended up writing "settled overnight" several million times a year.

This module is not about writing more. Most services already document too much and communicate too little. It is about writing the things that turn out to matter, in a form that survives being read by someone who was not there.

How this module works Read the content straight through. This is a longer module and takes most people around ninety minutes. Then complete the quiz at the end. You need 80% to pass, and your certificate appears on screen immediately. The quiz is open book, there is no time limit, and you can reattempt as many times as you need.

Currency statement and scope This module reflects the position as at August 2026. It applies across Aged Care, Disability, Mental Health and Community Services, because the principles do not change between them.

Your employer's documentation policy is the operational authority for what you record, where, when and in what system. This module teaches the reasoning underneath those policies so you can apply them well, including in the situations no policy anticipated.
